I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of my application for a pet puppy.

All pet puppies sold as companions are sold under contract to be spayed (girls) and neutered (boys) by 8 months of age. Proof of spay / neuter must be provided to breeder. Photo of vet report as well as photo of surgical site are required. By checking this box, you are agreeing to this policy.

I am applying for the Guardian Home program - the above policy does not apply to my application.

Upon approval of this application, I understand that a $500 CAD deposit will be required to hold my spot for a puppy. The deposit will be deducted from the final total cost of my puppy.
